🏡 Adult Family Homes (AFH)
Adult Family Homes provide housing and 24-hour care for up to eight adults in a warm, residential setting. These smaller, home-like environments are ideal for older adults who need more personalized assistance than what’s typically available in Assisted Living or Memory Care communities.

🌿 What Adult Family Homes Provide
AFH's can accommodate individuals with higher levels of care, often comparable to skilled nursing — except for IV therapy.
Common services include:
-
1-2 person transfers and Hoyer lift assistance
-
Full support with bathing, dressing, mobility, and toileting
-
Management of bladder and bowel incontinence
-
Assistance with meals and feeding
Many homes are owner-operated, and every provider is a Certified Home Care Aide (HCA) at minimum, as required by Washington State law. Some owners are also licensed nurses, offering additional clinical oversight.
🏠 The Living Environment
Residents enjoy their own private or shared bedroom and share inviting common spaces such as the living room, dining area, and kitchen — creating a warm, family-style atmosphere.
🩺 Move-In Requirements
Before moving into an Adult Family Home, Washington State law requires a Nursing Assessment by an independent, state-licensed RN Assessor.
This one-time evaluation determines the appropriate level of care and typically costs $500–$600.

🌸Pros
-
Highest staffing levels — typically 1–3 caregivers for every 6-8 residents, allowing for more individualized attention, and even 1 on 1 care if needed.
-
Provide care through the end of life and partner closely with hospice providers.
-
Rarely require residents to move out for higher levels of care; most can remain in the home even as needs increase.
-
Most homes have a visiting “House Doctor”, nurse practitioner, or podiatrist who provides care onsite.
-
Many are flexible with finances, offering Medicaid spend-down options and admits.
-
Long-term staff retention promotes consistency and familiarity in care.
⚠️Cons
-
Can be more expensive than Assisted Living initially due to higher staffing ratios and smaller capacity.
-
Less privacy and fewer planned activities compared to larger communities.
-
No transportation services are generally provided or an additional charge.
-
May not accept residents with severe behavioral issues, as these can be disruptive in intimate home settings.
💰Costs
-
Average monthly rate: $7,500 – $12,000 (all-inclusive).
-
Month-to-month agreements are standard.
-
Many homes accept spend-down to Medicaid, and Long-Term Care insurance and Veteran's Aid & Attendance may be used.
